Decongestive Lymphatic Therapy
Decongestive Lymphatic Therapy (DLT) is a comprehensive treatment approach that refers to a number of ways in
which a qualified lymphatic practitioner can support a client with managing and reducing a lymphoedematous
swelling, and continue to support the long-term management of lymphoedema, aiming to improving quality of life
and minimise symptoms. There are two phases to DLT, the Intensive Therapy (Phase 1) and Maintenance Therapy
(Phase 2).
DLT includes a variety of treatments, depending on an individual’s need at the time. This may include:
Manual Lymphatic Drainage Massage (MLD). This is a gentle, specialised massage that stimulates
the lymph flow around the body by stimulating/opening the lymph nodes and manually moving
fluid towards the nodes to drain away from the site of accumulation.
Compression bandaging involves applying specific elasticated bandages in a particular manner, to reduce
swelling, improve circulation and also support the healing of certain wounds. This ultimately reduces the
size of the limb and encourages drainage of stagnant lymphatic fluid from the affected limb
The wearing of medical compression garments which involves being measured and fitted for the
correct garments in order to put a controlled level of pressure on the legs to reduce swelling and
improve circulation. This can also reduce the pain and discomfort caused by lymphoedema
Medical devices may also form part of the therapy, and there are many available. These may include a sequential intermittent pneumatic compression pump which involves wearing an inflatable sleeve on the affected areas(s) that inflates and deflates to enhances lymphatic flow and supports with decongestion, or a low level laser which is a non-invasive pain free treatment that helps to remove excess fluid from the cells and reduce tissue break fibrosis.
Health and lifestyle advice – diet, exercise, skincare and advice around activities of daily living which are all important in lymphoedema management. Referral to other healthcare providers may also be offered where necessary
